3. NSO Authenticated Birth Certificate (photocopy)
4. Marriage Contract (If married)
5. Police Clearance/Barangay Clearance (for male students)
6. Entrance Test Result for Placement
7. 1”x1” Picture for Examination Permit
8. 2”x2” Picture for the Guidance Office File
Additional Admission Requirements for Nursing Program
1. Stool Exam
2. Urinalysis
3. CBC
4. Chest X-Ray
5. Hepa Screening
6. Medical Certificate
Academic Standards
- Students Academic Load
- Regular student - 18 and above number of units
- The number of loads allowed per semester as indicated in the course curriculum.
- Students should take the required subjects by curriculum year. Advance subjects may be taken only after passing the basic and corresponding pre-requisite subjects. Any subject taken ahead of its pre-requisite will not be given any credit and the student will be required to repeat the subject. read more »
