Requirements for Transferees

  1. Certification of Transfer/Honorable Dismissal
  2. Certification of Good Moral Character
  3. Official list of subjects taken and units earned/ transcript of records from last school attended
  4. Entrance test result for placement
  5. Birth Certificate (photocopy)
  6. Marriage Contract (if Married)
  7. d. Police Clearance/Barangay clearance (for male students)

    Students who stopped schooling other than disciplinary or academic problems may be readmitted.

Requirements for Returnees

  1. Updated Transcript of Records
  2. Honorable Dismissal
  3. Certificate of Good Moral Character

Requirements for Summer

  1. Summer Permit from College Dean/Registrar of last school attended
  2. Certificate of Good Moral Character
  3. Police Clearance/Barangay clearance (for male students)

Additional Admission Requirements for Nursing Program

  1. Stool Exam
  2. Urinalysis
  3. CBC
  4. Chest X-Ray
  5. Hepa Screening
  6. Medical Certificate
